Recruitment Forms

Membership is refreshed each year to avoid the Panels becoming
unrepresentative over time and to give as many people as possible the
opportunity to take part. If you are a resident in Carmarthenshire,
Ceredigion or Pembrokeshire and would like to make your voice heard
about local services, plans and proposals, please complete our
recruitment form.

Working for Dyfed-Powys Police, the Council, Hywel Dda Health Board or Pembrokeshire Coast National Park Authority does not exclude you from joining the Panels, however if you are a member of senior management or the Police Authority, we would ask you not to request membership due to the seniority of your position within the organisation.

will be kept for 2 years, will only be used for Citizens' Panel
purposes and will be treated with complete confidentiality under the
Data Protection Act i.e. it will not be shared with third parties. The
Data Protection Act 1998 regulates the collection, storage,
processing and disclosure of personal data and also gives rights to
individuals about whom information is recorded.
