Making a Data Protection Request

Subject Access

One of the rights of a Data Subject.

The Subject Access provisions of the Data Protection Act gives an individual the right to have a copy of any personal data held about them, subject to certain exemptions.

Section 7 of the Data Protection Act 1998 states that a “data subject” (the person about whom the personal data refers) is entitled, upon written request, to be informed whether or not personal data is held or processed about them.

Making a Request

Should you wish to know what information (if any) Dyfed-Powys Police holds about you, or you would like confirmation that you do, or do not have a prosecution/conviction history; you must complete a Subject Access form.

Completed application forms together with the £10 fee and appropriate identification (please see form for details), can be returned by post or handed into any Police Station in the Dyfed-Powys Police area. We are unable to accept completed forms by e-mail.

Please note that once we have received your completed application, the process to provide you with information can take up to 40 days. If you live abroad and need proof that you do not have a criminal record you should make a Subject Access enquiry with the police force where you were last resident.

Filling in the Form on the Data Protection

The Data Protection Act 1998 (DPA 98) gives you the right to access personal information (information about yourself) held by Dyfed Powys Police and any information held on the Police National Computer.

Request for information from the Police National Computer
Complete ACRO SAR 1 Form

You should use this form if you need a check to be made against your conviction history records. This will include court convictions, police cautions/warnings, reprimands and Penalty Notice for Disorder [PND]. This form must be printed and completed following the instructions contained on the form. It must be sent direct to ACRO - address ACRO (SAO) PO Box 638, Fareham.

The processing of these applications is carried out by the Association of Chief Police Officers Criminal Records Office [ACRO] on our behalf and all queries about how the process works, timescales for responses should be directed to ACRO, SAO) PO Box 638, Fareham Tel: 0845 6013999.

Subject Access Form is available for download.  Details of how to pay and what to include are found on the form. All cheques to be made payable to Police and Crime Commissioner for Dyfed Powys Police.

If you do not have access to a printer you can contact us (via the contact details on this page) and request us to send you the form via the post.

Form 3

Request for both information from the Police National Computer and Dyfed Powys Police please follow Form 1 and ACRO will forward your application to Dyfed Powys Police for processing.
